INCOMING STUDENT-ATHLETES

SPORTSWARE APPOINTMENT DIRECTIONS 

AFTER MAY 15

  1. All new freshman or transfer Student-Athletes must create a profile in SportsWare (Sportsware Directions)
  2. Complete all 5 SportsWare information tabs (general, address, emergency, insurance, medical)
  3. Upload into SportsWare a copy of insurance card, front and back
  4. Complete 3 SportsWare required forms (Health Information Portability and Accountability Act, Athletic Insurance Requirement Form, Assumption of Risk Form)
  5. Complete SportsWare medical history
  6. Watch sickle cell trait info video, upload completion certificate, and sign declination form, OR upload test results. The Sickle Cell Test Declination Form can be found under the athlete portal
  7. Contact your coach for instructions on baseline concussion testing
  8. The NCAA requires a physical dated within 6 months of the first day of practice. Athletes participating in fall sports must have physical dated after 2/15, winter sports 4/15, spring sports 3/15 of the current year. Upload the physical form into SportsWare. Blank forms came be downloaded HERE.
  9. IF YOU TAKE MEDICATION FOR ADD/ADHD please have your doctor fill out this form and submit it, and the supporting documentation.

When you arrive at school, you will need to check in with the sports medicine staff. If you have not completed all the necessary steps, you WILL NOT be permitted to begin practice with your team.

Please note that the requirements for campus are different and separate than the requirements for participation in AMCAT athletics.

If you will be under 18 when you move onto campus, please make sure your parent or legal guardian also signs all forms.

 

RETURNING STUDENT-ATHLETES

  1. Log into SportsWare, www.swol123.net. Update any information within the 5 tabs that has changed in the last year (general, address, emergency, insurance, medical)

Sickle Cell Information

The NCAA mandates that all Student-Athletes be aware of their sickle cell status. Student-Athletes at Anna Maria College have the option to either provide proof of sickle cell testing or waive the requirement.  If a Student-Athlete chooses to waive testing, they are required to watch the educational video on Sickle Cell, pass a quiz, upload the certificate AND sign a waiver. If Student-Athletes want to provide their test results they can get a blood test from their primary care doctor or provide the results of testing that was done at birth.

Banned Substances

It is the Student-Athletes responsibility to check all over-the-counter medications, prescription medications, nutritional/dietary supplements (this includes multi-vitamins, single form vitamins, protein powders, creatines, pre/post workout, weight loss), etc. through Drug Free Sport to ensure it is safe and will not lead to a failed drug test. The National Center for Drug Free Sport Inc. provides information on banned substances and dietary supplement information.
