Prior to participating in Anna Maria College athletic teams, athletes must provide the Sports Medicine Department with several pieces of information such as current address, emergency contact, health insurance, medical alerts and health history information. To expedite this process, we use an online electronic medical record. Please do not sign up for sportsware prior to May 15.
