Below you will find information about 2023 fall preseason move in dates and information for all athletes regarding sports medicine, athletics communications, and NCAA Compliance Paperwork for 2023-2024 athletic participation. Please pay careful attention to each area and complete all necessary paperwork. If you have any questions, please do not hesitate to reach out to the appropriate person (see contacts below).  

PRESEASON MOVE IN INFORMATION (FALL SPORTS)
Preseason camps are invitation only.  Invited student-athletes will receive information from the Head Coach and Athletic Department with more details about preseason camp.

SPORTS MEDICINE PAPERWORK (DEADLINE, AUGUST 1, 2023) (ALL SPORTS)
The Anna Maria College Sports Medicine department utilizes the EMR, SportsWare Online for medical information for its student-athletes. Each student-athlete must complete a profile in SportsWare Onine, including demographic information, emergency contacts and insurance information. All student athletes must submit a physical from within 6 months of the start of their season.

All freshmen, new athlete participants, and transfer student-athletes must also submit sickle cell trait blood test results, and complete a medical history. Instructions for the computer-based concussion baseline testing will be emailed to each new student-athlete once their SportsWare Online requirements are complete, including submission of their insurance information, physical, and sickle cell test results (first-year students only).

FINANCIAL OBLIGATIONS
You will not be permitted to move on campus or participate in any preseason activities unless all of your financial obligations have been fulfilled. You must complete all of your financial aid documentation and you must be in good standing with Student Accounts. You will receive updates on your financial status throughout the summer. Please contact Student Accounts at (508) 849-3425 or the Office of Financial Aid at 508-849-3366 if you have any questions regarding your account.

SUMMER ORIENTATION PROGRAMS
Orientation attendance is required for all new students. New students from Massachusetts, New Hampshire, Maine, Connecticut, Vermont and Rhode Island must attend an Orientation program, June 20-21 or July 10-11. 

Students from outside of the New England States, including International students and transfers are welcome to register for a June or July session if this is convenient. If you are not in the New England states, an Orientation Session will be hosted August 23-24.  
 
The next important step in your enrollment at Anna Maria is attending an orientation session, and there a few options of when you can attend. Click on the session/date that is most convenient for you to register:

June 20-21 Session – Register
July 10-11 Session – Register

COLLEGE MEDICAL HEALTH FORM REQUIREMENTS INCLUDING COVID VACCINE INFORMATION

Anna Maria College and the Commonwealth of Massachusetts require health forms and immunization records to be submitted via our secure patient portal, Medicat, no later than July 31st for Fall Semester enrollment. Failure to comply with this deadline will be considered a violation of the student code of conduct and will hinder your ability to register for classes or move onto campus. If the required information is not received by Friday, October 20, 2023  a registration hold will be placed on your student account and you will not be able to register for classes.

To complete required health forms and submit required documents:
1) Go to the Medicat Patient Portal log-in page: annamaria.medicatconnect.com
2) Register for the portal following the directions on the registration page
3) Once registered, log-in to your Medicat portal account
4) Follow the instructions on the Medicat homepage for completing forms and uploading documents

Please check your Anna Maria College email (@amcats.edu) on an ongoing basis for communication from Health Services. You will be contacted via email if information is incomplete or missing, and when you have completed the requirements.

Please remember all forms must be submitted via the Patient Portal. No forms will be accepted by mail, email, fax, or in person.
